Résumé: La radiothérapie exclusive est une technique éprouvée dont les indications sont aujourdhui exceptionnelles et réservées aux situations palliatives particulièrement chez les patientes âgées inopérables. La radio–chimiothérapie concomitante ou séquentielle est une technique éprouvée qui améliore le taux de réponse histologique et le taux de conservation mammaire particulièrement utile dans le traitement des cancers localement évolués ou inflammatoires. Cependant le taux des complications postopératoires et la médiocrité du résultat esthétique pondèrent lintérêt de cette technique. Il faudra reconsidérer cette technique si lhypothèse du relargage de facteurs de croissance par le geste chirurgical initial, source des récidives précoces, se trouve confirmée dans lavenir.

Breast cancer. Neoadjuvant treatments. Neoadjuvant chemotherapy or neoadjuvant radiochemotherapy
Abstract: Primary radiotherapy is efficient, but today this technique is reserved for palliative settings notably for old, fragile patients. Concomitant or sequential radiochemotherapy are two efficient tools to treat locally advanced or inflammatory breast cancers. Poor cosmetic results and the rate of surgical complications need too be very carefully considered when indicating these techniques. If the hypothesis of growth factor redistribution by initial surgery is confirmed in the future, this technique could constitute a very interesting means to lower the rate of early relapses.
